Anyone who wishes to be a head or assistant coach must complete this Form or have completed it in prior seasons. All who coach will be checked for compliance.
Return or mail the completed Background Check Authorization Form, to Tallowood Basketball, 555 Tallowood Drive, Houston, TX 77024 as soon as possible. Your actual signature is required.
